Sometimes when you interact with the TV in the Dream World, you get the Aztec Video instead of the Eye. I'm not sure exactly what causes the Aztec Video to appear. I've tried a lot of things to see if I can find anything that causes it to no avail. It may be possible that you must be holding all 24 Effects for it to happen, but I'm not even sure about that. I have tried loading a saved game where I have all 24 Effects, doing something and then getting the Video; then reloading the same game and doing the exact same thing again and I don't get the Video. So I really don't know. (And yes I have stood there and flipped the TV off and on for a long time; I do think doing something or one of a number of things in the game triggers the Video.)
Note: Visitors to the site report it seems to be completely random, so that's probably it. You can try waking up, going to sleep, and interacting with the TV repeatedly to try to trigger it if you want to see it.
